This condition, which often causes pain in the lateral wrist and thumb area, is particularly common among individuals who regularly use their hands for tasks such as texting, typing, or playing sports. Middle-aged individuals, especially Black females, pregnant women, and those with diabetes or rheumatoid arthritis, are more prone to developing this condition. At our office, we take a comprehensive approach to diagnosis, beginning with a thorough patient history. This allows us to accurately identify the condition and customize our diagnostic tests. One of the primary tests we perform is Finkelstein's test. During this test, the patient extends their arm, tucks their thumb into their palm, and makes a fist. We then apply pressure to the thumb, and if pain occurs in the wrist or thumb, this indicates a positive result. Another test we use is the hitchhiker sign test. Here, the patient raises their thumb as if hitching a ride, and we apply pressure against it. Increased pain in the lateral wrist or thumb signifies a positive outcome. We also perform the "what's test," where the patient rotates their hand with an open palm and extends their thumb. If pain is felt during this maneuver, it further confirms the diagnosis. Finally, palpation is a simple yet effective method for detecting pain or inflammation. By gently pressing on the affected area and noting any clicking or popping sounds when the thumb is moved, we can confirm the diagnosis.
